Current Pay Stubs (covering the past 4 weeks)
If your employer offers health insurance, the amount it would cost each month to cover your child or children
Your child’s proof of citizenship or residency
Your child’s Social Security number OR
If your child has not yet received a Social Security card, the date you applied for it, OR
Your child’s USCIS (immigrant visa) number
