Children’s health insurance is now available to every school-age child in Florida who needs it. Every single one. Our price is based on your income. That’s one reason most Florida children are insured these days. Our streamlined application process makes it easy to apply online. It takes most people less than 20 minutes from start to finish.
Here’s what you need to have handy to get started:
                 Income information
Your family’s most recent tax return (Form 1040) OR
Your Wage and Earning Statement (W-2 Form) OR
Current Pay Stubs (covering the past 4 weeks)
If your employer offers health insurance, the amount it would cost each month to cover your child or children
Your child’s proof of citizenship or residency
Your child’s Social Security number OR
If your child has not yet received a Social Security card, the date you applied for it, OR
Your child’s USCIS (immigrant visa) number
